Your natural beauty begins with a confident smile.
Rebuilding Your Smile’s Foundation
Over time, missing teeth or gum disease can cause the jawbone to shrink or the gums to recede, exposing roots and compromising aesthetics. Bone grafting involves placing specialized material in areas of bone loss to stimulate your body to regenerate its own bone. This is often essential for creating a solid base for dental implants or for preserving the jaw structure after an extraction.
Similarly, tissue grafting is used to restore gum tissue where it has receded, covering exposed roots and reducing sensitivity. These regenerative procedures are key to both function and beauty. They restore the natural contours of your jaw and gums, protecting your teeth and ensuring that any restorative work, like implants or crowns, looks completely natural.
When Dental Bone Grafting or a Gum Graft Is Recommended
Grafting is not a treatment on its own. It rebuilds the foundation for something else. Common reasons include:
- Too little bone for an implant: After long-term tooth loss, gum disease, denture wear or injury, the ridge can become too thin or short.
- Upper back teeth near the sinus: A sinus lift adds bone below the sinus floor.
- Preserving a socket after extraction: Graft material placed when a tooth is removed can limit the shrinkage that follows.
- Bone defects around natural teeth caused by periodontitis.
- Gum recession: Exposed roots, sensitivity, or thin gum that keeps receding.
Grafting may not be advised while gum disease is active, in heavy smokers, or where health conditions impair healing. Alternatives can include shorter or angled implants, a bridge or denture instead of an implant, or desensitizing treatment and monitoring for mild recession.
Bone and gum grafts solve different problems. A bone graft restores volume inside the jaw, mainly so an implant can be placed or a tooth’s support improved. A gum graft thickens or repositions the soft tissue on the outside, mainly to cover roots and stop recession progressing. Some patients need both, at different stages.
How Bone and Gum Grafts Are Placed
- Assessment: Gum measurements and digital X-rays show how much bone or gum is missing and which graft type fits.
- Numbing: The area is fully numbed with local anesthetic.
- Access: The gum is gently lifted to expose the defect or exposed root.
- Grafting: For bone, the space is filled with graft material and often covered with a protective membrane. For gum, a thin piece of tissue (or a donor substitute) is positioned over the root.
- Closure: The gum is stitched so the graft is stable and protected.
- Review: A check-up and stitch removal, then monitoring until the site is ready.
According to the American Academy of Periodontology, bone built by a ridge augmentation or sinus lift usually needs about 4 to 12 months before an implant goes in. Sometimes the implant can be placed at the same visit.
Healing After a Graft
Expect swelling and soreness for several days, sometimes with bruising. If tissue was taken from the palate, that area is often the more tender spot. Keep to soft, cool foods, chew away from the site, avoid pulling your lip to look, and do not smoke. Small granules of graft material in the mouth in the first days can be normal.
After a sinus lift you will be asked not to blow your nose forcefully and to sneeze with your mouth open.
Call the clinic if bleeding does not stop with pressure, swelling or pain worsens after day three, a large amount of material comes out, or the stitches open. Fever with facial swelling, or difficulty swallowing or breathing, needs urgent care.
What Affects the Plan, Cost and Outcome
- The size and shape of the defect, and whether it is a bone or gum problem
- Graft material and whether a membrane is needed
- Number of teeth or sites treated
- Whether the graft is done with an extraction or implant, or as a separate stage
- Smoking, diabetes control and oral hygiene
Results vary between individuals. A Cochrane review found that socket preservation may reduce ridge shrinkage after extraction, but the evidence is uncertain and no single graft material proved clearly better. A healed bone graft generally behaves like your own bone, while a gum graft holds up best when the brushing habits or gum disease that caused recession are addressed. Occasionally a graft does not take fully and a second procedure is needed, which we will discuss with you in advance.
