Difficult Tooth Removal

Safe Management of Complex Extractions

While our primary goal is always to save teeth, there are times when removal is necessary for your health, such as with impacted wisdom teeth or severely broken roots. “Difficult” removal refers to teeth that are not fully erupted or have complex root shapes. We approach these procedures with a focus on surgical precision to minimize trauma to the surrounding bone and gum tissue.

Your comfort is paramount during these procedures. We utilize advanced local anesthesia and gentle techniques to ensure you feel no pain. Following the removal, we provide detailed guidance and care to promote quick healing. Our expertise ensures that even the most challenging extractions are handled smoothly, paving the way for future restoration or immediate relief from pain.

When It Is Needed

When Wisdom Tooth Removal or Surgical Extraction Is Advised

Surgical extraction is used when a tooth cannot simply be lifted out. The most common example is an impacted wisdom tooth, one that is blocked by the tooth in front or partly covered by gum. Removal is usually advised when it causes:

  • Repeated gum infection around a partly erupted tooth (pericoronitis)
  • Food trapping, decay or gum disease affecting the tooth or its neighbor
  • A cyst or abscess

Other difficult cases include teeth broken off at the gumline, roots left behind after a previous fracture, and teeth with curved or very long roots.

A wisdom tooth that is healthy and causing no problems is usually left in place and monitored at routine check-ups, rather than removed just in case.

Step by Step

How a Surgical Extraction Is Carried Out

  1. Imaging and planning: Digital X-rays show the angle of the tooth, the root shape and how close it lies to the nerve in the lower jaw.
  2. Local anesthesia: The area is fully numbed; extractions at the clinic are carried out under local anesthesia. If you are very anxious, tell the dentist beforehand so the visit can be paced around you.
  3. Access: If gum covers the tooth, a small flap is opened, and a little bone may be removed to reach it.
  4. Sectioning: The tooth may be divided into two or three pieces so each can be removed with less force on the surrounding bone.
  5. Cleaning and closing: The socket is cleaned and the gum is closed, often with dissolvable stitches.

Most surgical extractions are completed in one appointment and you go home the same day. Very deep or high-risk teeth may be referred to an oral surgeon.

Recovery

Recovery After Wisdom Tooth Surgery

Recovery takes longer than after a routine extraction. Swelling and soreness are usually worst over the first two to three days and then improve. For up to two weeks it is common to have some bruising, a stiff jaw and discomfort when chewing. Some people need a day or more off work.

Helpful habits include taking pain relief as advised, sleeping with your head raised, eating soft food, rinsing gently from the second day, and avoiding smoking and alcohol while the socket heals.

Contact the clinic if pain gets worse rather than better a few days after surgery (a sign of dry socket), if bleeding will not stop, or if numbness of the lip, chin or tongue continues after the anesthetic should have worn off. Fever, spreading swelling or difficulty swallowing or breathing needs urgent care.

Planning and Risks

What Affects Your Surgical Plan

  • How deeply the tooth is buried and its angle
  • Root shape and how near the roots lie to the nerve or the sinus
  • Whether infection needs settling before surgery
  • Your age and medical history, including blood thinners
  • Whether one or several teeth are removed at once

The risks are explained before you agree to treatment. Besides dry socket and infection, a lower wisdom tooth that sits close to the nerve carries a small risk of numbness or tingling. This usually improves, though recovery can take weeks or months.

Compare Options

Surgical Versus Routine Extraction

A routine extraction removes a fully visible tooth in one piece with no cuts to the gum, and most people are back to normal the next day. A surgical extraction involves a gum flap, sometimes bone removal and sectioning, and stitches, so it brings more swelling and a longer recovery. The X-ray assessment shows which approach your tooth needs.

General & Preventive Dentistry

Other Treatments We Offer

Oral Examinations & Screenings

Prevention is always better than cure, and a comprehensive oral examination is the most effective tool we have to protect your smile.

Digital X-rays & Diagnostics

Many dental issues, such as bone loss, cysts, or decay between teeth, hide below the surface where the naked eye cannot see.

Tooth Extractions

Our primary goal is always to preserve your natural teeth, but there are times when an extraction is the healthiest choice-whether due to severe damage, advanced infection, or crowding.

Tooth Colour Fillings

Cavities weaken the structural integrity of a tooth, but treating them shouldn’t mean compromising your smile’s appearance.

Crowns & Bridges

When a tooth is severely damaged or weakened by a large filling, a simple repair isn’t enough to withstand chewing forces.

Dentures

Losing multiple teeth can have a profound impact on your ability to eat, speak clearly, and feel confident in social situations.

Fluoride & Sealants

Your tooth enamel is the hardest substance in your body, but it faces daily attacks from acids in your food and bacteria in your mouth.

Mouthguards & Nightguards

Teeth are incredibly strong, but they are not indestructible. Many people unknowingly grind or clench their teeth while sleeping (bruxism), leading to worn enamel, sensitive teeth, and chronic jaw headaches.

Bonding

Sometimes, a minor imperfection—like a small chip, a stubborn stain, or a slight gap between teeth—is all that stands between you and a perfect smile.

Teeth Cleaning

Even with diligent daily brushing and flossing, plaque can harden into tartar (calculus) in hard-to-reach areas, becoming impossible to remove at home.

Zoom Whitening

Over time, teeth naturally darken due to aging, dietary habits like coffee and wine, and lifestyle factors.
