Revascularization and Regenerative Therapy

Revitalizing Immature Teeth

When a child or teenager injures a permanent tooth that is still growing, traditional root canals can stop the root from developing, leaving the tooth weak. Regenerative endodontics is a cutting-edge biological procedure designed to replace damaged tissue with living, healthy tissue. Instead of just filling the space with rubber, we stimulate the tooth’s own stem cells to encourage continued root growth.

This therapy is a game-changer for young smiles. It aims to revitalize the tooth, often restoring sensation and strengthening the root walls to prevent future fractures. By harnessing the body’s natural healing ability, we can save young permanent teeth that would otherwise be lost, ensuring a lifetime of strength and function for your child.

Who It Suits

Is Regenerative Endodontics Right for Your Child?

Regenerative endodontics (also called revascularization) is used for permanent teeth whose roots have not finished forming, where the pulp has died after an injury or deep decay. The usual candidates are children and teenagers, roughly 9 to 18 years old, with:

  • A front tooth that was knocked or chipped and has since darkened or developed a gum boil
  • An immature tooth with an infection at the root tip
  • Thin root walls and a wide-open root end on the X-ray

It is not suitable if the root has already finished forming, if the tooth will need a post to hold a crown, if the child is allergic to the medications involved, or if the child cannot manage several visits. In those cases, apexification or a conventional root canal is used instead.

After any knock to a tooth, see a dentist promptly. A tooth that is knocked out needs emergency care straight away.

Two Visits

How Regenerative Treatment Is Carried Out

  1. Assessment: Clinical tests and digital X-rays confirm the root stage and the infection.
  2. First visit: Under local anesthetic, the canal is gently rinsed with disinfecting solutions, with little or no filing so the thin walls are not weakened. A medicated dressing is sealed inside.
  3. Interval: The tooth is left for about one to four weeks while the infection settles.
  4. Second visit: If the tooth is symptom-free, a small amount of bleeding is encouraged into the canal from the tissue beyond the root tip. The blood clot acts as a scaffold for new tissue.
  5. Sealing: A biocompatible material is placed over the clot, and the tooth is closed with a bonded filling.

If infection persists at the second visit, the dressing step is repeated.

Aftercare

Recovery and Follow-Up

The tooth may be tender for a day or two. Your child should avoid biting hard on it and should wear a mouthguard for contact sports. Call the clinic if swelling, a gum boil or pain returns.

Follow-up X-rays are usually taken at about 6, 12 and 24 months, then yearly. Signs of success are, in order of priority: no symptoms and healing bone, then thicker and longer root walls, and sometimes a return of sensation in the tooth.

Outlook

Limitations, Planning Factors and the Alternative

The plan depends on how developed the root is, the extent of infection, the child’s ability to cooperate and how many dressing visits are needed. Known drawbacks include discoloration of the crown from some medications and materials, calcification inside the canal, and roots that do not keep growing. Research shows the new tissue is mostly repair tissue rather than true pulp, which is why the main goal is a healthy, stronger root, not a fully living tooth.

The main alternative is apexification, where a barrier is placed at the open root end so the canal can be sealed. It controls infection but does not thicken or lengthen the root. If regenerative treatment does not work, it can be repeated or converted to apexification or a conventional root canal.

For Parents

Helping Your Child Through Treatment

Most children manage regenerative treatment well under local anesthetic, but the visits can feel long. It helps to explain the visit simply beforehand, avoid words like “needle” or “drill”, and plan a quiet afternoon afterwards. Keep every review appointment, even if the tooth feels fine, because changes in root growth or color only show up over time. If your child plays contact sports, ask about a custom mouthguard to protect the tooth while the root strengthens.

Other Treatments We Offer

Root Canal Treatment

Forget the myths you have heard; modern root canal therapy is a pain-relieving treatment, not a pain-causing one.

Root Canal Treatment Under Microscope

The success of a root canal depends entirely on how thoroughly the infection is removed from the tooth’s interior.

Complex Root Canal Treatments

Not all tooth roots are straight or simple; some have severe curves, calcified blockages, or intricate branching systems that make standard treatment difficult.

Repeat Root Canal Treatments

Occasionally, a tooth that was treated years ago may develop a new infection or fail to heal properly due to complex anatomy or seal leakage.

Non surgical root canal treatments

When a cyst or infection develops in the bone at the tip of a tooth’s root, many assume surgery is the only option.

Frequently Asked Questions

Who is this treatment for?
It is primarily for children/teens with immature roots whose teeth have been damaged by trauma or decay.
Can it make a dead tooth alive again?
It can restore sensation and blood flow in some cases, but the primary goal is to strengthen the root.
How many visits are needed?
Typically two or more visits to disinfect and then induce the bleeding for stem cell growth.
Is it painful for the child?
No. We use effective anesthesia. It is often less invasive than a standard root canal.
What if it doesn't work?
If regeneration fails, we can still perform a traditional root barrier procedure (Apexification) to save the tooth.
Is it safe?
Yes, it uses the body's own blood and biocompatible materials.
